Our practice operates in a hybrid model, offering both in-person and Telehealth appointments. In-person sessions are available at our Downtown Chicago office (53 W. Jackson Blvd), our Crown Point, IN office (710 Merrillville Rd), and our Mt. Prospect location (200 E Evergreen Ave., Ste 122, Mt. Prospect, IL 60056). Tesa sees clients in-person at our Indiana office, while Liz sees clients in-person at our Mt. Prospect location.
We are in-network with BCBS PPO and BlueChoice PPO. For other insurance providers, we are happy to provide detailed statements and assist with any necessary paperwork for reimbursement. Please visit our FAQs page for more information on how to check your out-of-network benefits.
