Telehealth Services Notice
Embody + Mind Collective, PLLC
Providing safe, convenient, and secure therapy

What Is Telehealth?
Telehealth allows you to connect with your therapist using secure video and audio technology. It enables the delivery of psychological services—such as assessment, diagnosis, treatment, consultation, and education—from a location of your choosing.

What You Need to Know
At Embody + Mind Collective, we are committed to ensuring your care is accessible and supported, whether in person or virtually. Please review the following important information about our telehealth services:

Your therapist may recommend telehealth sessions as part of your care.

Our standard practice policies—including the Notice of Privacy Practices, Informed Consent, and Practice Policies—apply to telehealth services. Copies are available upon request.

  • Telehealth sessions differ from in-person sessions, as you and your therapist are not in the same physical space.
  • Telehealth offers benefits, such as improved convenience, increased access to care, and the comfort of meeting from your chosen environment.
  • There are also potential risks, including technology interruptions, unauthorized access, and reduced audio or visual clarity. Either you or your therapist may decide to pause or reschedule a session if the connection is not clinically appropriate.
  • We comply with all applicable laws, including State of Illinois regulations and relevant licensing board standards and ethics.

If you have questions or concerns about telehealth, your therapist will take time to answer them and discuss any alternatives that may better support your needs.

You may discontinue telehealth at any time, without impacting your access to future care or services.

Our Platform: Telehealth by SimplePractice

We use Telehealth by SimplePractice, a secure, HIPAA-compliant platform designed for virtual therapy. It is simple to use—no passwords or downloads are required to join your session.
Please keep in mind:
  • This platform is not an emergency service. In the case of an emergency or crisis, you should call 911 or go to your nearest emergency facility.
  • Certain clinical situations may not be appropriate for telehealth. Your provider will work with you to determine the best format for care.
  • SimplePractice facilitates secure video sessions only; it does not provide medical services, clinical decision-making, or emergency response.
  • For your privacy, do not share your telehealth session link with anyone who is not authorized to attend your appointment.

If you have questions about telehealth or whether it is the right fit for your care, please don’t hesitate to reach out to our team at hello@embodyandmindcollective.com.
